Come join our team as a Registered Dietitian in our state of the art, skilled nursing facilities.  We are looking with someone who can float between our facilities to assist the dietitians within the buildings. It requires flexibility to travel to different facilities from Northern KY to the Dayton area. ThisThis positions functions in an administrative, scientific and educational capacity. The dietitian is to provide the best possible resident care through clinical nutrition management and individualized food service designed to meet the needs of the resident. The dietitian reports to the facility Administrator and is in close communication with the corporate dietitian.

RESPONSIBLITIES:

  • Performs Clinical Assessments, updates patient care plans and completes appropriate MDS related sections
  • Maintains a working knowledge of the Medicare PDPM system to ensure the facility is capturing all patient related needs
  • Documents in the facility electronic medical record (PointClickCare) and uses the facility meal planning software to ensure proper patient meal needs are in the dietary software
  • Works collaboratively with the facility chef and the dietary department to promote optimal food service operations
  • Attends meal services and takes an active role in participating and solving meal service issues. Completes meal rounds to effectively monitor resident meal intake. Assists with food service inservice training as needed.
  • Works with the facility to maintain dietary cost control by balancing appropriate nutritional care with use of supplements and other nutritional interventions. Self-monitors supplement usage via Supplement PPD.
  • Collaborates with patients, families, physicians and the IDT for Diet Liberalization: Aims for long term care patients to have self-directed nutritional care by utilizing Regular diets, providing real foods first and personal choice.
  • Works with physicians/physician extenders to communicate nutritional recommendations, weight changes and other areas as needed. Communicates nutrition issues to other disciplines and the facility administrator as they arise to effectively solve problems. Effectively informs resident and legal representatives of new orders, diet changes and weight loss notifications as warranted.
  • Maintains a professional/leadership Role by representing self as a leader in field of nutrition. Effectively solves problems related to nutrition and dietary. Works closely with food service manager to assist with food service issues. Has effective time management skills and reliable attendance. Able to function independently however builds rapport with other disciplines to be an effective team member. 

QUALIFICATIONS:

  • The dietitian must be licensed in the state in which she/he practices.
  • Detail oriented person who ensures processes are completed timely
