Job Summary:
The Providence Center Public Safety Case Manager will create, develop, support and maintain a relationship with Providence police personnel by performing a range of clinical and administrative functions under the direction of the Supervisor. Coordinate services for consumers to minimize stressors to facilitate the reduction of psychiatric symptoms by performing the following duties.
Duties and Responsibilities: include the following. Other duties may be assigned.
Provide and coordinate with fire, police and first responders for the provision of non-emergent street based outreach for individuals in the community experiencing a psychiatric symptoms.
Work closely with TPC Acute Care Programs – Emergency Services, Engagement and Diversion, Crisis Stabilization Unit and other teams as appropriate to coordinate care and services.
Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Facilitate collaborative relationship with the police department, the fire department and other first responders.
Serve as a community liaison for Community Diversion with community agencies.
Initiate treatment and advocate for consumer within network of care.
Schedule appointments for consumers within The Center and with outside agencies.
Assist consumers with benefits acquisition (social security supplemental insurance (SSI), food stamps, advocates office, medical doctor).
Organize and support the team’s daily delivery of service. Meet program goals/standards of care in accordance with Center policies and BHDDH regulations.
Recognize consumers in crisis and make referrals to community diversion team therapist, emergency services, or as appropriate.
Treat consumers with respect; promote their dignity, accept their values and lifestyles and maintain their confidentiality in accordance with Center policy and legal requirements.
Be flexible in regard to providing services during start-up phase and in ongoing staffing.
Requirements:
Associate degree required.
Bachelor’s degree (B.A.) from accredited four-year College or University in Human Services preferred.
Experience in Human Services Field preferred.
Bilingual Spanish preferred.
Must have valid driver's license and insured auto.
