Overview
The Graduate Nurse (LPN) Program is a comprehensive program for new graduate nurses with less than six (6) months of experience. The program serves to educate, train, support and evaluate nurses who have graduated from a NLN-AC or CCNE Accredited Nursing Program and are eligible to take the NCLEX exam within the next three (3) months. The Graduate Nurse provides treatments and services to patients in accordance with the state’s Board of Nursing regulations only under the guidance and supervision of a licensed Registered Nurse (RN).
Responsibilities
- Learns from and rides along with other licensed nursing professionals
- Understands how to conduct comprehensive medication reviews and ensure patients and their caregiver, if any, receive written information regarding the patient’s medication regimen
- Learns the processes of how to develop, implement and update individualized plans of care for patients in partnership with the patient, caregivers and physician, and identify patient-specific measurable outcomes and goals.
- Participates and successfully meets the requirements of critical thinking skills development
- Learns to provide services and treatments to patients that are ordered by the physician or allowed practitioner as indicated in the plan of care
- Gains knowledge of how to effectively monitor and evaluate patient response to treatment, medication and healing, and communicate with the patient’s physician or allowed practitioner regarding patient needs and changes in condition and obtain orders for changes to the plan of care
- Understands how a nurse provides patient and caregiver education on the patient’s diagnosis, medications, infection control, and safety measures in accordance with the plan of care in the Home Health Setting
- Learns to prepare clinical notes by documenting observations, treatment provided and patient response to treatment, and submit documentation to the agency in a timely manner
- Participates in the agency’s QAPI program by communicating problem areas identified and recommendations to address problem areas; data collection; attendance at periodic QAPI meetings; and participates in performance improvement projects as requested
- Attends and participates in patient care conferences, staff meetings, in-services and other meetings as requested
- Maintains universal precautions and infection control standards to prevent the transmission of infections and communicable diseases. Appropriately disinfects/sanitizes/transports equipment and supplies used for patient care; and uses proper medical waste disposal techniques
- Learns how to participate in the on-call rotation to cover patient needs after hours, on weekends and holidays
- Maintains confidentiality of patient related information and honors patient rights
Qualifications
- Recent graduate of an approved school of professional nursing-within the last six months
- Successfully passed NCLEX Exam within first ninety days of hire, at which time you will be considered for a position as a Full Time Licensed Practical Nurse
- Current CPR certification
- Basic clinical skills
- Professional appearance and demeanor
- General knowledge of computers and the ability to learn industry specific software applications
- Excellent oral, written, and interpersonal communication skills
- Excellent organizational and time management skills with a keen attention to details
- Ability to work effectively in a team environment
- Ability to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issues
