FULL TIME CLINICAL COORDINATOR- HOME AND COMMUNITY TEAM!
Who we are looking for:
- Are you the super-star multitasker type, who is passionate about treating clients and has strong leadership and administration skills?
- We are hiring a Clinical Coordinator who is a licensed PT, OT, SLP, or LCSW to provide coordination services for our rehab team
- The Clinical Coordinator will serve a dual role as the service coordinator and treating clinician on the team

Responsibilities
What you will do: Responsibilities listed include but not limited to:
- Formulates rehabilitation treatment plans, functional outcomes/goals and service delivery for referred patients in conjunction with the patient/support group and the clinical team.
- Develops patient initial, progress and discharge reports and monitors the patient’s status on an ongoing basis.
- Schedules/facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians and family members.
- Monitors projected hours of service to be delivered to the patient and reports hours/subsequent changes to the Director Clinical Management/Program Manager.
- Acquires and tracks orders from physician for all provided ongoing services after the initial orders are obtained.
- Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations.
Qualifications
What you will need:
- Minimum of Bachelor’s degree in Allied Health (Occupational Therapy, Physical Therapy, Speech Language Pathology)
- Maintains relevant professional licensure/registration/certification in the state of practice
- Minimum of two-year’s experience in a rehabilitation setting with neurologically impaired patients
- Prior case management and/or insurance administration experience preferred
- Demonstrates excellent organization, problem solving, decision making and communication skills
- Has reliable transportation and is able to travel to/from multiple assignments/locations on a daily basis
- Ability to lift 50 pounds
- Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
- Duties require fine motor skills,visual acuity, and walking/ standing for extended periods
- Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
