Essential Responsibilities
·Assist with resident financial paperwork and annual recertification.
- ·Provide input in the service planning process for each resident.
- ·Ensure that all resident records, reports, progress notes are informative and descriptive of the services provided.
- ·Assist with new resident orientation.
- ·Provide a smooth transition for residents moving to another level of care.
- ·Provide solutions for social and practical environmental problems including seeking financial assistance, financial planning, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
- ·Interpret and evaluate social, psychological and emotional needs of the resident/family to the nursing staff and other resident care team members.
- ·Coordinate psychosocial assessments for all residents.
- ·Establish and post a service listing for self-referral of residents.
- ·Participate with resident council. Provide assistance with and responds to written requests that result from the meetings.
- ·Facilitate the resident council.
- ·Facilitate communication with responsible parties, family members, facility, physicians’ offices, and local ombudsman as needed.
- ·Assist with coordination of resident service plans, advanced directives, and periodic review.
- ·Educate staff, residents and families on issues related to aging in place, the aging process and, related social service topics.
- ·Provide assistance in resolution of inter-resident conflicts and resident family difficulties.
- ·Adhere to established safety procedures.
- ·Assist staff and residents in emergency situations.
- ·Follow facility and organizational policies and procedures.
- ·Attend in-services as required.
- ·Provides excellent customer service to all who interact with the program.
- ·Perform other duties as assigned to help meet the goals of the program or the agency.
Essential Responsibilities – Accounts Payable:
- Processes account payable transactions.
- Review and verify invoices and check requests
- Sort, code and match invoices
- Enter and upload invoices into system
- Research and resolve invoice discrepancies and issues
- Maintain vendor file
- Duties include Accounts Payable for all billing and use of webserve
Qualifications:
- Bachelors Degree in Social Work, Human Services or related field
- Current knowledge of Social Security, Medicare/Medicaid and other insurance reimbursement regulations and policies as it pertains to Resident needs.
- Knowledge of supportive services and other resources for senior citizens in the local area
- Thorough understanding of the theory, practices and principals of social services.
- Ability to work independently and exercise good judgement/ decision-making.
- Ability to travel to various Ascentria and community partner locations with own vehicle is required.
- Ability to set priorities and manage multiple tasks while meeting deadlines.
- An understanding of the significance of confidentiality and agreement to maintain this policy in all areas of facility work and abide by HIPPA at all times.
- Computer skills required; including MS Word, Excel and Outlook.
- Ability to read, write and communicate effectively in English.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset people within the facility.
- Valid Driver’s License and proof of automobile insurance required.
