ESSENTIAL FUNCTIONS:  To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.  

• Assess, plan and provide a comprehensive plan of care that proactively manages healthcare issues and addresses barriers along the care continuum for patients in the nursing home setting

• Provide medical care within the scope of the practitioner’s training and consults with supervising physician when necessary and appropriate

• Ensure compliance with local, state, and federal regulatory agencies related to clinical services provided by NPs or PAs

• Assess and determine appropriate medical and psychological intervention (as applicable)

• Prescribe medications and services as indicated

• Order appropriate laboratory studies, diagnostic procedures, and consultations

• Monitor compliance with medications and evaluate patient response to plan of treatment; modify plan of treatment when necessary

• Work with RN Case Managers as part of an integrated care team (as applicable) 

• Perform comprehensive risk assessment per Model of Care

• Participate in the development of an Individualized Care Plan; attend interdisciplinary Team Meetings as needed

• Actively manages Advance Care Planning for assigned patients

• Complete initial, annual, change in condition, and post-hospital history and physicals

• Attend weekly case conferences

• Engage key stakeholders to facilitate care delivery consistent with our Model of Care

• Maintain thorough and timely documentation of services rendered in the patient’s chart

• Provide complete and correct billing information on all patient visits, consultations, and procedures to office staff (as applicable)

• Assist with on-call service with physician staff to provide 24/7 access to care for patients

• Practice in a cost-effective manner

• Participate in the development of policies and procedures relating to the provision of clinical and related services

• Participate in the review of quality related data that is related to clinical practice and outcomes

• Conduct informal facility staff education and teaching to improve the effectiveness and efficiency of care delivery

• Perform administrative and other duties as required

Job Requirements:

• Must be able to practice and lead effectively in a collaborative practice model

• Ability to effectively communicate and interact with multicultural and multi-age populations with a strong focus on customer service

• Broad knowledge of medicine and care of specialty related populations

• Ability to adapt to change and manage time efficiently

• Understanding of current recognized standards of care within the scope of licensure

• Thorough knowledge of applicable state and federal laws and regulations

• Self-directed and able to work with minimal supervision

• Reliable attendance

• Reliable transportation

• Current automobile insurance according to company policy

• Successful completion of require training

Required Work Experience:  

• Prior primary care or internal and/or geriatric medicine preferred

• Minimum of 3 years of experience in clinical nursing or rehabilitation for the geriatric population preferred

Licensing/Certification/Education Requirements:  

• Bachelor’s Degree for PA 

• Master’s Degree’s for APRN and/or CNS 

• Graduate of an accredited APRN or PA program  

• Nationally recognized certification

Required Computer Software/Equipment Used:

• Microsoft Office including Outlook, Excel and Word

• Standard office equipment

• EHR experience 

• Microsoft Teams experience preferred

Supervisory Responsibilities: 

• May be required to provide training and advice to facilities

• Occasionally needs manager’s direction due to extraordinary circumstances
