JOB SUMMARY:
The Case Manager is responsible for developing and implementing individualized service plans for individuals and their advocates within a residential setting. This includes providing case management services, identifying appropriate service providers, and ensuring the satisfaction of individuals with the services provided. The Case Manager will also participate in interdisciplinary planning and coordination of services.

SCHEDULE: This is a full-time role(Monday-Friday 9AM - 5PM)

COMPENSATION: $27/hourly

The position is grant funded through September 30, 2026.


ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide social services to residents, enhancing their social functioning and safeguarding their human and civil rights.
- Develop and monitor individual treatment plans in accordance with regulations and agency policy.
- Interpret and follow up on recommendations from interdisciplinary team meetings, and chair these meetings.
- Collaborate with residence and professional staff to interpret and explain psychological needs.
- Develop goals and activities generated by the interdisciplinary team, and produce a treatment plan schedule for each individual.
- Document comprehensive functional assessments and review plans of care as needed.
- Ensure proper data collection through observation and in-service training.
- Assist in activities of daily living and participate in activities outside of the residence.
- Work with our coaches and residence program specialists in implementing active treatment plans.
- Regularly communicate with families, residents, and agency-wide contacts.

QUALIFICATIONS:

- Exceptional interpersonal skills to effectively relate to individuals at all levels and be sensitive to program needs, employee goodwill, and the public image.
- Strong negotiation and communication skills.
- Passionate, highly motivated, and able to generate excitement and interest in the organization.
- Strong initiative, eagerness to learn, and ability to work collaboratively and independently.
- Ability to maintain confidentiality and prioritize tasks.
- Ability to travel to different locations and administrative sites as required.

EDUCATION and/or EXPERIENCE:

- Bachelor's degree or higher, plus a minimum of three years of experience working with individuals with developmental disabilities.
- Experience in providing parenting skills training services.
- Familiarity with New York City's child welfare and intellectual and developmental disabilities systems.

LANGUAGE SKILLS:
- Ability to read, write, speak, and understand English; bilingual Spanish speaking preferred.
- Excellent written and verbal communication skills.
