Functional Pathways is currently hiring at The Christian Village at Mt. Healthy in Cincinnati, OH for a Full-time Wellness/Activities Coordinator.

Previous Wellness experience required. 

Job Responsibilities:

  • Report to and work very closely with their designated Regional Manager
  • Works in conjunction with Functional Pathways Corporate Wellness Director; communicates with on a regular basis
  • Maintaining an open line of communication and pro-active relationship with your facility personnel and therapy group to ensure your satisfaction with your wellness program and services
  • Attending all rehab-related meetings in the facility
  • Ensuring all fitness assessments and evaluations are performed promptly
  • Assuring that your wellness program meets all needs, including development of specialized wellness and marketing programs
  • Plan, organize, and lead wellness activities on and off campus
  • Coordinate and oversee implementation of wellness programs
  • Gather information on resident lifestyles and goals
  • Lead wellness for all levels of care
  • Refer patients to therapy as needed
  • Develop relationship with patients newly released from therapy
  • Obtain all prior information needed from patient and pharmacy for evaluation process.  
  • Gather all patient information packets, and obtain signatures before evaluation/treat process.
  • Obtain doctor’s orders/fax completed evaluation paperwork for signature on timely basis.
  • Maintain patient records, and print weekly documentation to be faxed to the corporate office weekly for billing purposes.
  • Ability to work under limited supervision and able to lead, guide, and/or coordinating others; and directing the use of budgeted funds within a work unit.
  • Utilization of resources from other work units is often required to perform the job's functions.
  • Preparing and maintaining accurate records; operating standard office equipment including using specific software applications, Internet and email
  • Perform basic math, including calculations; read technical information, compose a variety of documents, and/or facilitate group discussions; and solve practical problems.
  • Obtain knowledge based competencies required to perform the functions of the job including health standards and hazards, safety practices and procedures.
  • Participates in workshops/meetings for the purpose of conveying and gathering information required to help improve the wellness program.
  • Track and trend all wellness data, including but not limited to, fitness scores, attendance, and continuum of care.

Requirements:

  • Associate's degree or higher from accredited institution preferred, Exercise Science and/or Sports medicine related field; and experience coordinating Wellness Programs
  • Two years of experience planning, teaching, and motivating individuals
  • Two years of experience in working with the elderly population
  • Must possess characteristics such as; good written and verbal communication, innovative, a self-starter, and team player
  • Must hold current BLS certification; preferred ACSM certification
  • Fluent providing Exercise Prescriptions and conducting individual and group sessions on topics related to all dimensions of wellness
  • Yoga certification strongly encouraged/preferred
