The pay scale starts at $14.00 and above in consideration of experience.

Citizens offers a variety of residential supports based upon the level of support and supervision required by each person to include Individualized Residential Alternatives (IRA’s) and Intermediate Care Facilities (ICF’s).

Qualifications:

  • High School Diploma or GED
  • Valid New York State Driver’s License that is also qualified in accordance with agency’s insurance standards; successful completion and maintenance of AMAP status
  • Ability to communicate and understand written and oral directives/information and an excellent team player with a positive attitude.

Primary duties and responsibilities include but not limited to:

  • Reviews medical appointment calendar on a daily basis to see what appointments are scheduled for the day
  • Accompanies and transports people supported on medical appointments and supports person to advocate for their medical needs
  • Confirms location of appointment, obtains vehicle for appointment, obtains directions to appointment and allows ample time for timely arrival at appointment
  • Ensure the needs of the person supported are met before, during and after appointment. This includes but is not limited to hygiene, toileting, meals, snacks and medication administration
  • Ensures that all needed information for successful completion of the appointment is in hand prior to leaving for appointment (i.e., consultation form, medical book, nurses notes, etc.)
  • Obtains completed consultation form and needed scripts from the physician prior to leaving appointment and delivers them to the house nurse upon return
  • Performs all other duties as requested by supervisor

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check.
