Discharge Advocate (LPN)

We’re seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our team as a Discharge Advocate, helping patients navigate the critical transition from hospital to home. If you thrive on connection, coordination, and clinical insight, this role offers the chance to make a real difference—right from your desk.

What You’ll Do

  • Connect with patients within 72 hours of discharge to ensure their recovery is on track and their questions are answered.
  • Ask the right questions—especially for those at high risk of readmission—to assess needs, clarify instructions, and identify gaps in care.
  • Support medication understanding and discharge instructions, offering clarity and reassurance.
  • Coordinate follow-up services when needed, working within the patient’s insurance or financial situation to ensure continuity of care.
  • Leverage your knowledge of community resources to guide patients toward helpful support systems.
  • Document and report using electronic medical records and databases, with both computer entry and face-to-face communication.

What You Bring

  • Education: Active LPN license required.
  • Experience: Acute care experience preferred.
  • Skills:
    • Exceptional phone communication and customer service.
    • Strong understanding of community resources.
    • Proficiency in navigating medical records and data systems.
