This is a non-exempt position that will be responsible for the coordination of medical services of individuals receiving residential and outpatient services, including but not limited to administrative assistance to attending physician and coordination of self-administration of medication..  The program operates twenty-four hours a day, seven days a week.  All activities within this position are completed in accordance with agency policy and procedures, Department of Children and Families (DCF), Thriving Minds South Florida Behavioral Health Network, Inc., federal, state and local laws, accreditation standards, other regulatory guidelines and tenets of the Catholic Faith. The Incumbent in this position demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.

Duties and Responsibilities:

Medical

  • Ensures services are provided in compliance with licensing funding and Catholic Charities Policies and Procedures.
  • Completes prescreen of referred consumers and initial medical assessments during consumer intake process.
  • Confirms with physician any allergies reported by consumer and ensures kitchen staff is notified of any allergies.
  • Schedules applicants and assisting the physician assessment to include lab screenings, physiological and/or psychiatric assessment, medical and psychiatric history and medication assessment and needs.
  • Provides rapid HIV testing with pre and post counseling session.
  • Organizes needed medical and other supplies in consultation with the physician and program director.

Medication

  • Coordinate with physician to ensure complete documentation of individual medical files.
  • Coach, counsel and motivate consumers to practice proper medication management practices.
  • Adhere to the program’s medication administration policy and procedure and monitor documentation of behavioral health technicians.
  • Ensures that consumers’ medications and logs are organized in an efficient manner to ensure that consumer may self-administer their medication and documented accordingly.
  • Review Medication Administration Record regularly for proper documentation and to report any medication errors to Program Coordinator.
  • Schedule all consumers at St. Luke’s residential facility for follow-up appointments with physician for medication management and medication tolerance and/or efficacy.

Consumer Services

  • Provides of medical case management and assisting program with maintaining integrated health services at the program.
  • Ensure that all the needs of our consumers are recognized and are met in a coordinated manner and that there are minimal gaps in services provided.
  • Assist in coordination and provision of the medical and psychiatric services provided at the program, including physical exams, laboratory screenings, treatment planning, discharge and aftercare planning relating to the client’s medical services.
  • Assist with consumer SOAR applications (SSI/SSDI Outreach, Access) and provide care coordination of medical and psychiatric services to consumers.
  • Completes all funder required documentation for consumers and submits for billing in a timely manner.
  • Facilitate for consumers the Hazelden Living in Balance Supplemental Sessions that pertain to health, wellness, and medication management.
  • Compiles and records information in the client record to ensure that it is in compliance with COA, Florida Statute, funding regulations and programmatic standards.
  • Secures authorization for release of information of medical and or other records in compliance with all state, federal and programmatic rules, and regulations. Ensures that pertinent information is logged before being mailed.

Risk Management and Quality Assurance

  • Comply with all policies, procedures, and requirements necessary to perform the functions of this position.
  • Participates in the Performance Quality Improvement process.
  • Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.
  • Ensures appropriate reporting of incident reports in adherence to DCF and Catholic Charities Policies and Procedures requirements.

Other Duties

  • Attends and facilitates training, meetings and conferences required by the funding source and/or the agency.
  • Adheres to the dress code, wears appropriate clothing and closed toe shoes, and follows safety protocols per OSHA and Catholic Charities when conducting job functions.
  • Perform additional responsibilities as required by supervisor.

Physical Demands:

  • The work is usually sedentary, but may require extended periods of standing, walking and/or bending and some light lifting.
  • Knowledge and practice of universal exposure control precautions.

Educational & Experiential Requirements:

  • High School Diploma or GED.
  • 1 year of relevant experience
  • Knowledge of psychiatric medication preferred.
  • Experience or training as a medical assistant preferred.
